Service Description
Use this request form to surrender campus-owned computers or printers that are no longer being used.
Standard Features
Standard Service Features include:
- Choose to drop computer/printer off at the IT Help Desk or have it picked up.
- Computer hard drives will be removed and destroyed.
- Computers will be delivered to Business Services.
Optional Features
There are no Optional Service Features associated with this service.
Roles and Responsibilities
User Responsibilities:
- User is responsible for providing details related to the request including their contact information, device names, serial numbers and location information.
Information Technology Responsibilities:
- Information Technology staff will assist with the surrender and surplus of the campus-owned devices.
- Information Technology staff will remove and destroy hard drives prior to surplus.
- Information Technology staff will communicate the status of the request to the user as needed.

Response and resolution times are based on operational hours (Monday through Friday from 8:00AM to 5:00PM).

Please Note: Response Time is measured from ticket submission date/time until the ticket is moved to an 'In Process' status. Resolution Time is measured from ticket submission date/time until the ticket is completed/resolved. A ticket in an 'On Hold' status (e.g. IT is waiting for additional information from the requestor) does not count toward the Resolution Time.
